Important! Some providers within the Partners HealthCare System are entering patient data using the Epic application. This may mean that the lists displayed in the LMR (including problems and medications) may not be up to date.
The View Partners eCare Chart button is now highlighted in pink if the patient's most recent visit was with a provider using Epic and if you have not yet clicked the button.
Once you have viewed the patient's Epic chart, you can request a CCDA document to allow you to reconcile problems and medications between the LMR and Epic. Do so by clicking Request PeC CCDA. For details, click here.
You can now also reconcile LMR medications with information gathered from multiple sources, including not only the patient’s Epic record, but also data from pharmacies and insurance claims data. Do so using the Consolidate Med List Viewer (CMLV) link from the Medications view. For details, click here.
Note:
The label "[uncoded]" next to medication names indicates entries that are not coded in the LMR database.
