Any LMR user can request a CCDA to reconcile LMR data to information in the patient's Epic record by clicking Request PeC CCDA. This button only displays if the following are true:
You previously viewed the patient's Epic record.
There are problems or medications to be reconciled.
A CCDA has not been requested for the patient within the last 24 hours.
When available, CCDAs can be viewed by clicking the Document to Reconcile link displaying in the banner at the top of the Medication, Problem, and Allergy main screens. The link also displays on the Clinical Reconcile view from the Patient Chart menu. To reconcile problems and medications, do the following:
1. Click Document to Reconcile.
2. In the column on the right, labeled "Sections to Reconcile," click either Problems or Medications. The LMR displays a split screen, with entries from the LMR on the left and entries from Epic on the right.
3. To verify entries on the LMR list, click Verify. For details, click here.
4. To remove entries from the LMR list to match the Epic list, click either D/C for medications or Inactivate for problems. For details, click here.
5. To add entries from Epic to the LMR, click Add. For details, click here.
6. To indicate entries from the Epic list that you will not be adding, click Don't Add. For details, click here.
7. If you are done making updates, click the check box, "I certify that the LMR (Medication or Problem) List is complete to the best of my knowledge."
8 Click Ok (ALT+O).
