Reconciling Medications

You can reconcile medications in the LMR in many different ways. Select the way that is most appropriate for your needs:

• Reconcile Medications based on Direct Patient Feedback

  Use this method if you are verifying medications based on direct communication with the patient or based on a report or form marked up by the patient. You should periodically review all medications on a patients medication list. For details, click here.

• Reconcile Medications based on Comments from Your Support Staff

  Use this method if your staff uses the Review Med List module to communicate updates from patients to providers. This feature is used by a limited number of practices (such as the MGH Cancer Center). For details, on incorporating Review Med List Requests, click here. • Reconcile Medications with Discharge Medications

  Use this method to reconcile the medications list for a patient with a recent admission to the BWH (up to 5/30/2015) or MGH. For details, click here.

• Reconcile Medications with Medications in the Patient Epic Record (Clinical Reconciliation)

  Use this method to reconcile the medications list for a patient with medications in recorded in a patient's Epic record (if applicable). For details, click here.

  Use this method to reconcile medications from an external document (such as a CCDA) with the LMR Medications list. For details, click here.

  Use this method to reconcile medications from multiple sources, including not only the patient's Epic record, but also data from pharmacies and claims data. For details, click here.
