Reconciling Medications

Note: The LMR may also include a button for Discharge Reconciliation. The Discharge Reconciliation button allows you to reconcile inpatient medications with the LMR medication list. The steps here allow you to reconcile the LMR list with medications the patient indicates that he or she is currently taking. If the patient has a Discharge Reconciliation button, completing that reconciliation first may save time. For details, see Reconciling Individual Medications.

You should periodically review all entries on a patient's active medications list. The following steps allow you record whether or not the patient is taking a medication or taking a medication differently than prescribed. Reconciling a medication updates the Last Updated column on the main Medications screen.

1.   If you haven't already, go to the Medications module.

2.   Click Med Reconcile. The LMR displays a new screen. This screen initially displays all active medications, sorted alphabetically. However, you can re-sort the list by clicking By Practice, By Drug Class, or By Reconciled Date.

3.   For each medication that you have not already renewed, edited, or discontinued, assess its status as follows:

   -  D/C - Use this status to remove the medication from the patient's active list.

   -  Not Taking - Use this status to indicate patient noncompliance without removing the medication from the patient's active list. This may be appropriate if you are not the prescribing physician and you would prefer just to flag the medication. The LMR allows you to type an explanatory comment (does not print on the prescription).

   -  Edit - Use this status to update the medication on the patient's active list.

   -  Taking Diff. - Use this status to indicate how the patient is taking the medication differently without updating the medication from the patient's active list. This may be appropriate if you are not the prescribing physician and you would prefer just to flag the medication. The LMR allows you to type an explanatory comment (does not print on the prescription).

4.   For practices set up as PCHM locations, click the check box to attest that the list is complete, if applicable. Note that you can only save this update if you have made other updates in step 3.

5.   If you want to save and sign your work now, click Ok & Sign. The LMR indicates the changes that will be saved, and allows you to electronically send or print prescriptions, if applicable (see Related Topics). Type your key and click Sign.

6.   Depending on your set up, you may be prompted to select a cosigner or sign for a provider. See Selecting a Cosigner and Signing and Saving for a Provider.
