A “Document to Reconcile” link and banner displays at the top of the Medication, Problem, and Allergy main screen if both of the following are true:
External documents (such as CCDAs) have been received within the past 180 days.
At least one external document includes a section that has not been certified as complete for the associated module (Medications, Problems, or Allergies).
To reconcile the LMR list to the list in any external document, do the following:
1. If you haven’t already, select the patient, go to the associated LMR module, and click Document to Reconcile. The LMR displays a list of all external documents received within the past 365 days. The right side of the screen includes links for Medications, Problems, and Allergies sections for each document. Links display in bold if the associated section has not yet been certified as complete.
2. In the column on the right, labeled “Sections to Reconcile,” click the module you want to reconcile. The LMR displays a split screen, with entries from the associated LMR list on the left and the list from the external document you selected on the right.
3. Work with entries from the LMR list as follows:
To verify entries, click Verify. For details, click here.
To remove entries, click either D/C for medications or Inactive for problems and allergies. For details, click here.
4. Work with entries from the external document as follows:
To add entries from the document, click Add. For details, click here.
To indicate that you will not be adding an entry, click Don’t Add. For details, click here.
5. If you are done making updates, click the check box, “I certify that the LMR (Medication, Problem, or Allergy) List is complete to the best of my knowledge.”
6. Click Ok (ALT+O).
