The color scheme used throughout the application has been updated. Additionally, button and tab placement and style have been modified to improve usability and consistency.
For practices participating in the Smoking Cessation study, the following icons now display, as appropriate, to the right of the patient name:
- Indicates that this patient is identified as either a current
smoker or former smoker who has quit less than three years ago.
Smoking information is recorded in the LMR via either an entry
on either the Problem List or the Health Maintenance grid. Clicking
this icon displays one of the following:
- For current smokers, displays the Smart Form for managing the care of patients who are smokers.
- For recent quitters, displays fields for updating this patient's smoking status on the Health Maintenance grid.
- Indicates that the smoking status has not been documented for
this patient in the LMR (via either an entry on either the Health
Maintenance grid or Problem List). Clicking this icon displays
fields for updating this patient's smoking status on the Health
Maintenance grid.
You can now view and act on clinical messages sent to others within your practice. Previously, you could only work with messages in your personal queue. See Working in Another User's Queue for details.
When replying to a clinical message, you can simultaneously send and delete the original message. Previously, doing so required two separate actions.
You can now view demographic
information for any patient associated with a clinical message.
To do so, click the
to the right of a patient name.
Printed clinical messages now include the message subject. Previously, the subject was not included on printed copies.
When adding or editing a note, you can now select to discontinue multiple medications simultaneously. Previously, you could only discontinue a single medication at a time.
When adding a section to a structured note or note template, the list of available templates has been reorganized for ease of use.
The note header has been modified to include the patient's date of birth. Previously, the date of birth was not included.
For users working with form-based
notes, you can now edit existing patient data (such as medications,
allergies, and problems) when adding or editing notes.
Click the
to the left of any item to display the associated entry form.
An Error button displays for all of your Preliminary status notes, allowing you to remove notes you've entered in error. For additional information, see Error Out an Existing Note.
The structured note default font size has been reduced to allow more of your note to be displayed at one time.
When adding or editing a note, the line separating the left and right panes of the screen has been made more prominent. By hovering your mouse over the bar, and then clicking it when it turns to double arrows, you can drag this bar to increase the size of the text entry window.
You can add problem-based plans to any free-form or structured notes (pilot users only). These plans guide you through the process of documenting the assessment and plan portions of a traditional SOAP note. For details, click here.

Diabetes and CAD Smart Forms will be released to pilot practices starting in November.
Smoking Smart Form will be available to randomized clinics to study usability and efficacy prior to general release. In addition to the smoking Smart Form, the following additional features have been added to aid in decision support and smoking cessation documentation:
- Reminders to document smoking status
- Updates to the smoking status entry on the Health Maintenance grid to allow for additional fields to be documented (such as cigarettes per day)

The Physician table on the Summary2 screen now automatically includes PCP and referring MDs if available from BWH scheduling systems. This physician list is primarily used to select providers who should be copied on notes during the CC List process for document distribution. For additional information on using CC lists, see Working with CC Lists.
You can now create notes for multiple patients from a patient list. Previously, you could only create notes for a single patient at a time. For details, see Creating Notes for Multiple Patients on a Patient List.

Pediatric Asthma Action Plan (to pilot)
The LMR now includes a Pediatric Asthma Action Plan to allow you to generate printed guidelines to help manage a pediatric patient's asthma. The printed documents are intended to be provided to the patient's parent or guardian, and can be based on any of the following:

Predicted peak flow
Personal best peak flow
This functionality is being piloted at selected Partners practices.
The Patient Assessment section has been updated for CTC (Common Toxicity Criteria), version 3.0. Values for the previous version (CTC version 2.0) were from 0 - 4; values for CTC version 3.0 are from 1 - 5. Numerous changes to labels and fields are also included. Additionally, you are prevented from importing information from a previous visit if that visit was based on a previous version of the assessment.
Visit information edited or updated after midnight of the day following the visit date is now flagged with an E (for edited), A (for addenda), or R (for revised), as appropriate. See Viewing a Flowsheet for details.
Whenever you add a new visit to the infusion flowsheet, the LMR now checks to be sure you are associated with the same institution as the infusion flowsheet. Users from one institution are no longer able to create a visit on another institution's infusion flowsheet.
You can now include oncology staging data items (from the TNM flowsheet) in custom queries.
Staging modules have been added for several new tumor types, including cervix uteri, endometrial, fallopian tube, germ cell, lung atypical carcinoma, lung carcinoma, lung - large cell neuro endrorine carcinoma, ovarian, vagina, and vulva. Additionally, changes have also been made to the staging module for the breast tumor type.
The EOL Referrals form has been updated to meet the requirements of those who use it.
The new Custom Menu option allows you to customize the menu options displayed within the LMR. You can hide menu or submenu items or move submenu items from one menu to another. To reset your menu at any time, click Reset to default. See Customizing Your LMR Menu for details.
This Signature Initiative program provides access to a personal health coach for patients from MGH, BWH, and NSMC who have no insurance or who are covered by Medicaid, Free Care, or dual coverage (Medicaid/Medicare). Health coaches can help patients manage their chronic medical conditions, better adhere to treatment plans, reinforce the importance of medication compliance and preventive care. To refer appropriate patients, click the MCaid HealthCoach option from the Patient Chart menu. For additional information, click here.
Automatic distribution of notes to CC list recipients is being piloted at selected pilot sites. For these sites, it is no longer necessary to click Distribute CC (in Document Management) to initiate sending to recipients set to receive via email, fax, or clinical message. For print recipients, a new Notes Comp Management report is available from the Tools option from the Admin menu.
Additionally, when the Carry Forward Last CCList option is selected (from Custom Preferences), a new "include cc list" checkbox is included on the edit screen for notes. This button allows the CC List to be suppressed for the current note only. This is useful for notes such as medication refills and telephone documentation which you might not want to send to all CC list recipients.
You can now view deleted immunizations by clicking Show Deleted. Previously, deleted immunizations could not be displayed.
You can now set a preference to print EOV forms when you are signing. This new functionality simplifies the process for printing forms.
Note: For users accessing the LMR on a computer with Windows XP, Service Pack 2 (either from a home computer through VPN or an office computer), additional steps may be necessary. These steps ensure that you properly download all necessary LMR note components. Click here for details.
