Working with Referrals Received

  • The Referrals Received view lists patients who have a referral sent to the service provider(s) you're authorized to see in 4Next.
  • To return to the list from any other view, click Referrals Received on the main menu. The data is refreshed automatically when you return to the list from any other 4Next view.
  • To receive email and/or text notifications when a referral arrives, or is confirmed, retracted, or finalized by the sender, or when a new message arrives, see  Setting up notifications. You must set up notifications for each site you're authorized to work with.
  • To set View By List: My List filter preferences, see Setting up your My List filter preferences. You must set up preferences for each site you're authorized to work with.

Filtering and sorting the Referrals Received listFiltering and sorting the Referrals Received list

You can set filters for this view to make it work the way you want. Your changes will persist while you're signed in to 4Next.

  • Status - By default, all of the active referrals to your selected site are listed, Active referrals include those with a status of New, Received, Accept Pending, Offered, or Accept Offer. See Referral Statuses.
  • To remove referrals with a particular status, click X on the Stat item in the search criteria.
  • To add referrals with a status that isn't in the search criteria, click to select from the status drop-down list. Selecting the Active option adds all active the status filters; selecting the Inactive option adds all the inactive (limited) status filters.
  • Inactive status filter limits: when you add an inactive status filter, the referrals that are returned are limited. Denied referrals are only shown if the associated case is still active, not if the case has been finalized or canceled by the sender. Retracted, canceled (by your site), and finalized referrals are limited to those that were inactivated within the previous 72 hours.
  • The only way to see inactive referrals that are outside the filter limits is to view by patient. See Searching for a patient history.
  • Site - When you first log on to 4Next, you must select a site to see the list of active referrals. This is required even if you have access to only one site. Once you set a site filter, the change is saved for the next time you log in. You can select only one (1) site filter.
  • To select a site, click to select from the site drop-down list. The My List filter for that site is automatically added as well. (See View by lists, below, for details on the My List filter.)
  • To switch to a different site, click to select from the site drop-down list; your new selection replaces the previous. The My List filter for that site is automatically added as well. (See View by lists, below, for details on the My List filter.)
  • You must also have at least one Status filter added to see the referrals from a selected site. 
  • The only way to see referrals for multiple sites is to view by patient. See Searching for a patient history.
  • View by list - The view by list options allow you to apply your My List preferences to your list of Referrals Received. If you have a single site displayed, additional view by list filters are available. You can apply your My List preferences in addition to a different view by list filter if you have a site filter selected.
  • My List with a site selected - this filter is always available, and is based on your Preferences setup. See Setting up your My List filter preferences. To limit the list to referrals that match your preferences for the selected site, click to select My List from the view by list drop-down list. 
  • My List with no site selection - this filter is always available, and is based on your Preferences setup. See Setting up your My List filter preferences. When you are searching a patient history, selecting My List from the view by list drop-down list limits the list to referrals that match your preferences for each of the sites you're authorized for. Your preferences for a site are applied only to the referrals belonging to that site.
  • Site-based - available when only 1 site is listed, these filters can be based on unread messages, case type, patient type or status, or care-related dates such as the expected/actual discharge date. The actual options available depend which type of site is displayed. To set a filter, click to select from the site drop-down list if needed, and then click to select from the view by list options. 
  • To apply your My List preferences in addition to a different view by list filter, click to select from the site drop-down list, and then click to select from the view by list options. (This option is only available when you have a site selected, even when you're only authorized for a single site.) 
  • To change a filter, click to select another option from the view by list drop-down list. In addition to the filter you select, your My List preferences for the selected site are also applied. 
  • To remove the selected filter, click X on the View by List item in the search criteria. In addition to the filter you remove, your My List preferences for the selected site are also removed. 
  • My referrals - Mass General Brigham staff who are authorized for case management in 4Next can be assigned as co-owners for any case at the site(s) where they're authorized. Click the My Referrals option on the my referrals menu to filter the existing list to display only referrals for cases that you own or co-own. See also: 
  • Sort - By default, the list is sorted alphabetically by patient name. 
  • To sort by a different parameter, click a column heading. All columns allow sorting.
  • To reverse the sort order, click the column heading again.
  • Sorting is disabled if there are more than 3,000 referrals in the list. Add filters to decrease the number of referrals to 3,000 or less to be able to sort the list.

  • All sites:
  • If there are any messages from the referral sender that haven't been marked as Read, the Unread messages heading column has a red circle with the total number of unread messages, like this: Unread messages flag.  If any of the unread messages is flagged as high priority, the ! column (on left) has a ! (red exclamation point).

  • If there are any documents from the referral sender that were shared after the referral was sent, the attachment column icon column has a link with the total number of new documents, like this: 2. Click the link to clear the counter and open the Referral Detail, where you can find the documents for the case at the bottom.

    **Whenever you open the referral details, the new documents counter is cleared.**

  • p/a - P= preadmission, A = ambulatory (from the case Details)
  • patient - last name, first name
  • dob - date of birth
  • insurance- provider of health insurance
  • from - site sending the referral
  • to - the site receiving the referral (shows only when there is more than one site displayed on the list)
  • disch. date - date of discharge: Exp= expected, home care only, pulled from the pre-admission analysis (PAA) form; Act = actual, pulled from the patient chart
  • program - pulled from the referral checklist
  • progressfrom the referral checklist, all providers: HOLD = patient med hold/delay; CLN = clinically cleared; home health & home hospice only: ORD = orders received; F2FHOLD = F2F hold/delay; Mass General Brigham continuing care only: REG = send to admissions initiated (this takes a few minutes after you click the button)
  • status - New, Received, Accept Pending, Offered, Confirmed, Denied, Canceled, Retracted, Finalized (See 4Next referral statuses); for RH referrals only, the status is red for Offered referrals with a screening form that is started but has not been signed within the last 48 hours (See Completing the screening form)
  • case owner - for the site sending the referral
  • Home care only
  • city, state, zip = displays the home care location when the service address is included in the pre-admission analysis (PAA) form; otherwise, the primary address (city, state, and zip code) from the patient record is shown
  • soc/roc - This column includes SOC (start of care), ROC (resumption of care), or Revisit date if entered on the pre-admission analysis (PAA) form
  • primary/discharge dx - from the pre-admission analysis (PAA) form, if available

Refreshing the list

The list is updated automatically when you return to it. This includes when you log on, and when you come back to the list from a different 4Next view. There is no timed refresh. Don't refresh the browser - that will set all your filters back to the default settings. 

  • To update the list manually without losing your filters and sorting setup, click refresh
  • Last updated is the time when the list data was most recently updated, either automatically or via the Refresh button.
  • After you click it, the Refresh button is disabled. It is automatically re-enabled after three minutes, or when you navigate back to the list from a different 4Next view.

Navigating the list 

Long lists are divided into pages. The page navigator includes a drop-down list of items per page drop down arrow, the current page number, the total number of pages, and the previous drop down arrow and next drop down arrow page buttons.

  • To change the number of items per page, click to select from the Items per page drop-down list (bottom right). 

  • To display the next page, click Right-arrow button. To display the previous page, click Left-arrow button. (Scroll up a bit if the page navigation buttons are covered by another button.)

  • Only 1,000 items can be displayed on the paginated list. (Printing the list is also limited to the first 1,000 items; sorting is restricted to 3,000 items.) If your list includes more than 1,000 items, you will only be able to page through to view the first 1,000. When you filter, every item on the list is affected, even if there are more than 1,000; however, only the first 1,000 items that match will be displayed. Use filters to limit the number of items to less than 1,000 if you need to view, print, or sort an entire list.
  • Your selection for Items per page will be the same the next time you log in.

Viewing details of a referral (quick view)

To display details on a patient and their referral, click the row (but not the name) for the case. Quick details include the patient's name, gender, date of birth and age, insurance(s), and the referring site and the receiving site. City, State includes the primary address (city, state, and zip code) from the patient record. For HHA referrals, the home care location is also displayed when the service address is included in the pre-admission analysis (PAA) form.

Accessing new documents

If there are any documents from the referral sender that were shared after the referral was sent, the attachment column icon column has a link with the total number of new documents, like this: 2. Click the link to clear the counter and open the Referral Detail, where you can find the documents for the case at the bottom.

**Whenever you open the referral details, the new documents counter is cleared.**

Selecting a referral to manage

  • To search for a particular referral:
  1. In the fields above the list, type the patient's last name, first name.
    Notifications include the initials for the patient’s first and last name, and their date of birth, like this: **A, H** 5/4/1986
    To find the related referral, type the initials with a comma and a space between.
    Compare the DOB as an additional identifier.
  2. Press ENTER. Any patients matching the criteria you entered are displayed in the list.
  • Click the patient name to select a case.

Printing the list

  1. Click print.
  • If there are more than 1,000 items in the list, you'll receive a warning message that the maximum number you can print is 1,000. Click cancel to close the popup message, and then filter the list down to 1000 items. 
  • The document opens in a popup window. From there, you can print or save a copy.

  • If there are multiple pages, every page of the list is included in the document. 
  • Every page includes a footer with "Extracted from the Legal Health Record and printed on " and the date and time stamp, plus "Mass General Brigham Incorporated Clinical data printed from 4NEXT - Confidential" and the page number and page count.

  1. Click × to close the popup.

  • If you need to print a list that has more than 1,000 referrals, try filtering by status, sites, and view by list. You can also set your advanced preferences to show names starting with A, select the My List filter, print, and repeat for the next letters until you've gone from A to Z.
