    To delete a Custom Form, click the Form filter (if necessary) to display a list of current forms. Then, click the .  The application displays a verification message. Click OK.

    Important: Use caution when deleting a Custom Summary. There is no verification prompt and you cannot undo this action.

    To delete a Custom Visit Summary, click the Visit Summary filter to display a list of current forms. Then, click the check box to the left of the form name and Delete.
