This option allows you to set the timing for sending forms to PG patients. You can select the visit/appointment types that trigger sending the form to the patient’s task list in PG. For all forms except Visit Summaries, you can also specify the timing for sending the form and reminder messages and restrict who receives the form by providers, age, sex, or problem list entries.
Note:
Appointment types must be defined before completing these steps. If you do not see all of your appointment types listed, contact your LMR analyst.
These steps are not required for forms that are to be printed and given to patients (that is, for forms not sent electronically).
1. Click the icon
associated with the form you want to configure.
2. For all forms except Visit Summaries, specify the number of calendar days before or after a visit you want this form to be available to PG patients.
3. For all forms, click to indicate the visit (appointment) types associated with this form. By default, all visit/appointment types are selected (that is, the form will be automatically generated for all visit/appointment types). Deselect any visit/appointment types that should not be associated with this form.
Note: If the header check box is selected (for all visit/appointment types) is selected, newly added visit/appointment types will default as selected. Otherwise, newly added visit/appointment types default as not selected.
4. For custom forms only, if you want
to restrict the sending of this form to patients who have visits with
specific providers, do so by click the name and .
By default, the form is sent to PG patients with a visit scheduled
with any provider from the practice.
5. For all forms except Visit Summaries, set the Message Parameters associated with this transmittal. You can specify the PG message sent to accompany this form as well as any reminders. For details, click here.
6. For all forms except Visit Summaries, if you want to restrict this form to specific patients, use the age, gender, and problem subset fields to do so.
7. Click OK (ALT+O).

If the patient electronically completes and submits the form, you can access it in the LMR, using the Patient Entered module, and choose to accept the input into the patient's chart or reject it. For details, click here.
