This module allows you to customize three different types of visit forms, represented by the links at the top of the screen: Forms, Visit Summary, and Standardized Forms. Depending on the type of form, you can customize the form contents, sending parameters (criteria to determine when the forms are sent to Patient Gateway users), or both.
Note: Not all forms are available to all practices. Currently, the Standardized Forms (including Asthma Control Test, Pediatric Symptom Checklist, and NICHQ Vanderbilt Assessment) are available to pilot practices only. Additionally, a link for External Forms displays for authorized Support Analysts only.
The Forms and Visit Summary categories allow you to customize content and even add new forms. Although both types of forms start with a standard list of sections, you can choose to suppress certain sections and reorder them. You can also customize the section labels, font size, contents, and page breaks.
All forms, including Standardized Forms, allow you to set sending (configuration) parameters for sending the forms to Patient Gateway (PG).
For the Visit Summary, you can specify the visit/appointment types that allow Visit Summaries to be automatically sent to a patient's PG account if not created within 3 days after a visit. You can also specify that default form used by your entire practice.
For Forms and Standardized Forms, you can specify not only the visit/appointment types that allows forms to be automatically sent, but also specify parameters to exclude forms from being sent (by age, gender, provider, and problem list entries). You can also specify the timing for sending forms as well as for sending reminders if the patient does not respond to the form.
