LMR Treatment Plan: The LMR (Longitudinal Medical Record) includes a section for oncology staging and treatment plan documentation. Providers can select a diagnosis and related regimen when completing the treatment plan. If a regimen is selected, the provider is prompted to select the chemotherapy medications that are included in the regimen and will be ordered for the patient. The diagnosis, regimen, and chemo medication options are drawn from the Chemotherapy Order Entry (COE) system. The selection of diagnosis, regimen, and medications for the treatment plan does not constitute an order, nor does it automatically transmit an order to the COE system. The information is for planning purposes only. Treatment Plan Prompt in COE: When the treatment plan in the LMR is finalized, the information is available in COE. Providers writing the initial set of chemo orders are prompted with the option to pre-fill the chemo diagnosis and regimen selection form with the information from the treatment plan. The selection is optional; the provider can opt to select any diagnosis and regimen as needed. Changing the Selection: The diagnosis and regimen can be changed after the treatment plan prompt. The prompt is for information and ease of use. Order Discrepancies: On completing the initial set of chemo orders in COE, if the diagnosis and regimen match the treatment plan, the chemo medication orders are compared to the information in the treatment plan. If the chemo medications from the LMR treatment plan do not match the chemo meds for the COE orders, the ordering provider is prompted, with the option to return to the orders to address the discrepancy. The provider has the option to continue without making any changes. Subsequent Orders: The prompt to select the diagnosis and regimen based on the patient's finalized treatment plan is presented for the initial set of orders only. Subsequent orders are automatically pre-filled with the diagnosis and regimen from the previous orders. Likewise, if a discrepancy exists between the chemo medications in the treatment plan and those in the actual orders, the information is presented only when the initial orders are written.
