Writing and discontinuing chemotherapy orders is restricted to authorized providers. See Overview of Writing Chemo Orders.
Discontinue any active medication order when administration must stop. The pharmacy cannot accept doses equal to zero (0). Do not set a dose to zero (0), or decrease a dose by 100%.
The affect of discontinuing a chemo order that has been signed—whether it has been activated or not—is to delete the scheduled days, not to omit or uncheck it on the order set. When ordering from previous, you must enter the days to administer that drug if you want to include it in the orders.
The orders generated by discontinuing active chemotherapy orders must be activated by a nurse. Check the activation date for your change order , and update if necessary to be sure that the order will be activated and carried out when you intend.
If you haven't already:
Click Write (ALT+W).
Click Chemo (ALT+H).
To access the LMR Oncology Infusion Flowsheet , click Infusion Flowsheet (ALT+F).
From the active Chemotherapy Orders list:
Click the line for the order to discontinue.
Click D/C (ALT+ D ).
When prompted, type the reason for discontinuing the order, then click OK.
If you decide not to discontinue the order, click Restore.
Discontinued orders can only be restored before signing. Once these orders have been signed, you must write a new order to restore them.
Click Close (ALT+C) to return to the session scratch pad .
You have the option to carry over discontinuation of active chemo orders to the existing orders that are awaiting activation in the future. Click Yes (ALT+Y) to do so when prompted, or No (ALT+N) to restrict the changes to the current active order.
Click Sign (ALT+S) and type your signing key when prompted.
Click OK (ALT+O).
You must notify nursing and pharmacy that you have changed an active chemo order. Change orders for chemotherapy must be activated by an RN before they can be administered. Click OK (ALT+O) to acknowledge the reminder message for this.
