To help clinicians recall what will be seen by the patient, each field
with information entered that prints on the Post-Hospital Patient Care
Plan (PHPCP) is flagged with the
icon. Please keep in mind that information on the PHPCP should be patient-friendly,
phrased to be easily understood by people who don't have medical training.
Face Sheet Discharge Orders and Instructions are not available in the newborn nurseries. You cannot access the forms, even if you select a patient from the associated maternity unit lists or from patient lookup.
If you haven't already:
Select the patient and click Face Sheet Discharge (ALT+F) or Discharge (ALT+G).
If needed, click the Order Info folder to access the form.
Complete the form:
As needed, select from drop-down lists to order the following diet and activity.
Type as needed to describe treatments. To work in a larger text area, click the field, then click Zoom. To return to the order, click OK.
Type as needed to describe additional orders, instructions, and equipment. To work in a larger text area, click the field, then click Zoom. To return to the order, click OK.
If needed, click to select TPN according to Nutritional Support Referral.
Type instructions for follow-up care as needed in the field for Doctor, Nurse Practitioner, or Physician Assistant Instructions for Follow-up Care. Include the date or time frame when applicable. To work in a larger text area, click the field, then click Zoom. To return to the order, click OK. This information is required to complete the discharge orders and prints in the Instructions at Home section of the Post Hospital Care Plan.
Post-discharge goals and care plan:
Continuity of Care is listed by default with the instruction to attend all post-discharge appointments; you can delete or edit this as needed.
To include additional goals and instructions, type in the next available line on the grid. Pressing the TAB key moves the cursor through the fields quickly. List each goal along with the instructions or intervention intended to achieve that goal.
All clinicians with discharge authorization can view the goals and instructions in a new Care Plan folder. To display, click the Multi-Disciplinary Care Plan link above the list, or click the Care Plan folder on the left.
This information is required to complete the discharge orders and prints in the Post Hospital Patient Care Plan.
Pending lab results:
Click to select the Yes or No option to answer "To your knowledge, does the patient have any pending results from chemistry, hematology, immunology, microbiology, pathology or radiology? "
If yes, then type or copy and paste the names of the tests.
To work in a larger text area, click the field, then click Zoom. To return to the order, click OK.
When you list pending tests, your entry and the contact information for finding results and reports is printed on both the Face Sheet and Post Hospital Care Plan.
If you would like to check your spelling, click Spell Check. When you have completed the check, click OK to close the Spell Check complete message.
Complete the Face Sheet, Medications, and Services forms if applicable. See also: Instructions for Discharge Orders and Instructions .
Save the changes, and finalize the orders if the patient is ready for discharge: see Signing Discharge Orders.
