View & print case documents & attachments

  • If you haven't already, from 4Next standalone:
  1. Click Cases Sent on the main menu.
  1. Click the patient name to select a case, and then click Details.
  • If you haven't already, from DHeCare (Epic Hyperspace):
  1. Log in to the Case Management or Care Coordinator module.
  2. From the patient encounter, click the 4Next tab.
  1. Click Details in the case navigation.

Viewing or printing a clinical document (Mass General Brigham inpatients only)

For Mass General Brigham patients only, clinical documents for the visit are available for viewing directly from the case. (The Documents section is not shown for OUT patients.) Clinical documents include case management notes, discharge information, the EMD (Early Maternity Discharge) form, the face sheet, the medications list, operative notes, PT/OT/ST (physical, occupational, and speech therapy) notes, and the referral summary. Any or all or none of the available clinical documents can be selected when setting up the referral, to be included when sending it to the chosen service providers. The documents are updated in real time.

  • The Documents section of the case Details allows you to view and print the clinical documents that are available from Epic. Some documents may be blank of they're not relevant to the patient. Documents and attachments are the last two sections in the case Details. Scroll down to view what's available.
  • Referral Summary from Epic: Information included
  • Discharge Info includes the Patient Care Referral Form (PCRF) with the Referral to Home Health HOMEBOUND Order (when applicable), Provider Discharge Summary, History, and Nursing Patient Assessment.
  •  To see what documents and attachments are shared with a provider, and to update what is shared:
  1. Click Manage to display the case management view.
  2. Click the Documents icon documents button on the provider card.
  • For online providers, you can update which clinical documents are shared, and allow sharing of all attachments that you add to the case. information in the clinical documents is updated from Epic in real time.
  • For fax-enabled providers, you can send or resend updated clinical documents and current attachments. Faxes of over 100 pages will fail; if you need to fax more than 100 pages, split them up into separate document sends.
  • To view an item:
  1. Click PDF print buttonView document button in the bottom left of its card. The document opens in a popup window.
  2. Click × to close the popup.

  • To open an item as a ready-to-print PDF:
  1. Click PDF print button in the bottom left of its cardThe document opens in a popup window. From there, you can print or save a copy.
  • Every page includes a heading with patient identification and case owner contact information.
  • Every page includes a footer with "Extracted from the Legal Health Record and printed on " and the date and time stamp, plus "Mass General Brigham Incorporated Clinical data printed from 4NEXT - Confidential" and the page number and page count.

  1. Click × to close the popup.

  • To print more than one item at a time, as a single PDF:
  1. Click Checkbox unchecked on the document card to select one or more; click Checkbox unchecked Select all in the Documents heading to check Checkbox checked every document. 
  2. Click PDF print button on the heading. 4Next collates the selected items into a single PDF. The document opens in a popup window. From there, you can print or save a copy.

  • Case Attachments are added and shared with the referral as needed. For OUT patients (non-Mass General Brigham patients), Case Attachments are the only documentation available to send with the referral.
  • To open an attachment as a ready-to-print PDF:
  1. Click PDF print button in the bottom left of its cardThe document opens in a popup window. From there, you can print or save a copy.
  2. Click × to close the popup.

  • To print more than one attachment at a time, as a single PDF:
  1. Click Checkbox unchecked on the document card to select one or more; click Checkbox unchecked Select all in the Attachments heading to check Checkbox checked every document. 
  2. Click PDF print button on the Document heading. 4Next collates the selected items into a single PDF. The document opens in a popup window. From there, you can print or save a copy.
  3. Click × to close the popup.

Returning to the patient's case management view

Click Manage.
