Attach, remove, or replace a document for a case

Paper documents can be scanned to create an electronic file, which can then be added to the case as an attachment. To protect PHI, save the PDF to your H (home) drive, and delete it once it's attached. 

  • If you haven't already, from 4Next standalone:
  1. Click Cases Sent on the main menu.
  1. Click the patient name to select a case.
  • If you haven't already, from DHeCare (Epic Hyperspace):
  1. Log in to the Case Management or Care Coordinator module.
  2. From the patient encounter, click the 4Next tab.
  • Click Manage to display the case management view.
  • Click Details

Case attachments, like visit documents, can be selected when setting up the referral, to be included when sending it to the chosen service providers.

  • To add a case attachment, the file must be saved as a PDF. You must have the file available to the device you're working on, and you can only attach one file at a time. 
  1. In the Case Attachments icon Case Attachments section, click Add Case Attachment button in the upper right. The Add Case Attachment popup opens.
  2. Click to select an option from the Category drop-down list.
  3. Click Choose File, and then browse to the .pdf file you want to attach.
  4. Click Open. The file name displays next to the Choose File button.
  5. Click submit.
  • To fax a paper form as an attachment to a case, you need to print a cover page to identify the patient and type of document. You can print the cover page to a printer or to a fax machine. A separate cover page is needed for each document, to identify which document is being attached. See Faxing a paper document to attach it to a case.
  • To remove a case attachment, click Add Case Attachment button in the bottom left of its card.

Returning to the patient's case management view

Click Manage.
