Canceling a case

  • If you haven't already, from 4Next standalone:
  1. Click Cases Sent on the main menu.
  1. Click the patient name to select a case.
  • If you haven't already, from DHeCare (Epic Hyperspace):
  1. Log in to the Case Management or Care Coordinator module.
  2. From the patient encounter, click the 4Next tab.
  • Click Manage to display the case management view.

Cancelling the case and retracting referrals that were sent

  • You can re-open a canceled case any time up until the discharge for the visit is complete.
  1. Click in the case type section in the upper right, and then click the Cancel Case icon Cancel Case option. The referrals that were sent are listed in a popup.
  2. Click to select an option from the reason drop-down list.

    Acute Reason for no Admission
     (finalizing/retracting referral)

    Definition

    Cancelled; Unknown

    Limited information on final dispo from referring facility

    Patient Expired or CMO

    Patient expired prior to Acute Discharge

    Patient Preference

    Patient selected another provider

    Referral Placed in Error

    Referral was an error ; wrong facility or patient

    Patient declined services

    Patient chose to be discharged without services

    Another level of care

    Patient discharged to a different level of care

    No services needed

    Patient did not need services

    Admitted/Transferred to another Acute service or Acute Hospital

    Patient is now at different level of care at the hospital

  3. If needed, type a message for the referral recipients.
  4. To flag the message as requiring immediate action, click Priority switch off to turn the Priority switch on: Priority switch on.

  5. Click send.
  • Each message is added to the applicable single-provider list and the Communicate message list. Messaging a provider automatically marks all messages from that provider as read Message read icon

  • The case is Inactive, with a Canceled status.
  • Inactivating referrals that were Sent flags them as Retracted icon Retracted. Inactivating referrals that were Drafts flags them as Removed icon Removed.

Re-opening the canceled case

  • When a case is canceled, all of the referrals that were sent are retracted, and any drafts are removed. While you can set the filters to see these inactive referrals, you can't add a need or re-send a referral until the case is re-opened.
  1. Look up the patient by name or MRN.

  2. From Cases Sent, to be able to select the patient, click the Canceled option under Inactive Cases on the status drop-down list.
