Working with Chart Lists

If applicable, a series of mini file folder tabs displays in the lower section of the Summary screen, in a multi-column layout.

Depending on the action you take from the Summary screen, temporary indicators will display to the left of the selected entry to let you know there is unsigned data.

• Click any underlined tab title as a shortcut to display the full module.

• For Problems, Procedures, and Family History

  Up to 20 entries are shown expanded (such as the 20 most recent problems). Use the scroll bar, if applicable, to display additional entries.

  There are a few different icons you may see displayed in these lists. (Click each graphic for a description.)

                                                

  Clicking this link displays general information on risk factors for the associated family history problem.

  An OnCall  icon displays to the right of a diabetic problem to allow you to access the OnCall Diabetes Monograph for the current patient. The OnCall Diabetes Monograph is a patient-specific “dashboard” of data and information related to a patient’s diabetic condition and care. It includes vital signs, relevant lab results, relevant medications, allergies, trend graphs against targets for certain data elements, and result-oriented reminders.

  Note: For the MGH Rheumatology location only, you can access the Rheumatology Summary Page by adding the OnCall tab to your Summary view. See Creating a Custom Summary for more details.

  A link to general information on problems or meds may be provided by the icon , if applicable. Clicking this link displays information from the MICROMEDEX or Skolar database.

  When the icon displays to the right of an entry, it indicates that a link exists between that problem or procedure and a note. This link is only established if you enter text in the free form or structured note related to the problem or procedure. You can click the icon to display the note.

  When this icon displays to the right of an entry, it indicates that a link exists between that med, problem, or allergy and a note. This link is only established if you enter text in the free form or structured note related to the med, problem, or allergy. You can click the icon to display the note.

• For Medications

  Same as above, with the addition of the following icons:

  • Indicates that the patient is not taking this medication (as documented during reconciliation)

  • Indicates that the patient is taking this medication differently than prescribed (as documented during reconciliation)

• For Immunization, Notes, Sticky Notes, Visits, To Do, Health Monitoring, Care Providers, Physicians, Advance Care Planning, and Patient E-mail

  Click the to expand the list and display associated entries. To hide entries, click the . (The LMR retains your show/hide settings for each tab for your next login.)

• Click Add New, if applicable, as a shortcut to display entry mode for the associated list. Alternatively, for some lists, you can directly add new entries. For details, click here.

  • For notes, click to select the type of note you want to add.

  • For selected lists (such as Problems or Medications), you can directly look up and add entries. Do so by typing three or more letters into the "Enter new..." field before clicking Add New.

• Click any underlined entry in a list to display it in edit mode.

Note: Your Summary screen may not include each of these tabs. You can customize your Summary screen using Custom Summary functionality. See Custom Summary Overview and Creating a Custom Summary for details.
