This release includes significant changes to the look-and-feel of the Notes module to streamline the documentation process.

The design of notes has changed to include an area to the left of the main note pane consisting of tabbed menus. From these menus, you can access information from other areas of the patient’s chart to view, update, and import into the note.
The Final, Setting (I/O indicator), Service, Specialty, and Training fields are now initially hidden from view and can be accessed through a nested menu on a new toolbar. The new toolbar also contains a Save, Save as Template, and Print Preview button along with the custom paragraph drop-down list.
You can resize the left and right panes using the panel splitter bar. Hover your mouse over the bar, and when it turns to double arrows, click and drag the bar to the size you want.
You now have the ability to add a new procedure or health maintenance item.
The following three additional error reasons have been added when erroring a note: Incomplete Note, Patient Cancelled, and Patient No Show.
Enhancements include the following:
The Document Management module has been updated to include a new toolbar for editing both free-form and structured notes. This toolbar adds options for envelope printing and text search. Previously, no toolbar displayed when editing structured notes.
Also, a new queue has been added for Scanning Unreviewed. This queue helps manage the workflow for practices that perform document scanning. It includes all notes that have been scanned for which you are identified as the author and for which the "Should be acknowledged" checkbox is selected.

Enhancements include the following:
- Milliliter and drop amounts default to the lowest amount available, even if it's a fraction.
- You can now multi-renew and multi-discontinue when working with medications in notes.
Scanning has been available in a limited way for several years. The underlying architecture has been enhanced to support a wider rollout across the Partners enterprise post pilot.
Enhancements include the following:
You can now create multiple personal lists from the My Patient List option from the Desktop menu. Previously, you could only create a single list using this feature. As a result, you now see a list of your current lists when accessing this option, rather than a list of patients. If you had previously created a personal list, it is assigned the name "My List conversion."
You can now create a letter for any patient on a list using any letter template. You can also print letters for multiple patients with a single button, or print a copy of any list.
You can also print a variety of reports for all patients on a list.
You can also automatically generate a list containing all patients meeting the criteria of any custom query (from the Custom menu). Contact your LMR support analyst for additional information.
The LMR now allows you to add annotated diagrams to free-form Notes. The feature provides a standard library of images along with the option to import your own digital images. Using graphic objects such as practice-specific markers or standard shapes, you can easily document information about a patient’s condition such as the anatomical location, size, color, or scale. This information is immediately accessible to all other LMR users.
Staging data is now available to be imported into a note.
You can now document Coronary Artery Disease, Diabetes, and Acute Respiratory Infections using the new Smart Form. This functionality is being piloted at selected Partners practices.
Smart Forms are LMR-based, condition specific applications that:
present and organize relevant patient data for each condition
facilitate documentation
provide one-click ordering of medications, patient education materials, and excuse-from-work notes (and soon, lab orders, appointments, and referrals)
By clicking on suggested orders, the Smart Form carries them out, updates the LMR, and documents the actions in the visit note.
Enhancements include the following:
- If you create the encounter in EOV, you can import into your note the orders, in-office procedures, diagnoses, or E & M level.
- In addition, diagnoses, orders, and procedures can be added to your note from your favorites, or you can add them from the associated dictionary. Adding any of these will automatically create an EOV encounter.
Note: For users accessing the LMR on a computer with Windows XP, Service Pack 2 (either from a home computer through VPN or an office computer), additional steps may be necessary. These steps ensure that you properly download all necessary LMR note components. Click here for details.
