1. If you haven't already, click MCaid HealthCoach from the Patient Chart menu.
Note: You can also click the MCaid Health Coach module from the Summary page if you have set this up in Custom Summary.
2. Click New Referral. The LMR displays the Health Coaching Request form.
3. Be sure all problems from this patient's active problem list that you want included in the referral are checked.
4. In the Most Appropriate Patients section, click one or more checkboxes describing the patient.
5. Using the drop-down list, select this patient's insurance status. This field is required.
6. Click to indicate whether you have discussed the benefits of health coaching with the patient.
7. Type additional information, as appropriate.
8. Click Ok (ALT+O).
9. If you want to save your work now, click Sign. The LMR indicates the changes that will be saved. Type your key and click Sign.
