Displays care providers that you or other LMR users have associated with this patient.
Click any line to display details about that provider. Click Add Provider to add an entry.
Displays physicians that you or other LMR users have associated with this patient. This table is also needed for CC list processing. Only physicians who are in a patient's physician table can be added to a CC list.
Click any line to display details about that physician.
Click Add Physician to add an entry by searching the physician database or to manually add a physician not currently listed in the database. For details, click here.
To add by searching the database, start by clicking the drop-down arrow at the bottom of the Physician screen to select a search based on name or license number. Type your search criteria and click Search. The LMR displays matching entries. Click to select a physician.
To manually add a physician, complete fields at the top of the Physician screen and click Ok (Alt+O).
Click Add PCP to add a patient’s PCP data from the EMPI. The button is active if 1) the patient’s PCP data is available from the EMPI and 2) the PCP is not already listed in the table of physicians.
For practices using automatic note distribution, click Clinic CC List to create a CC list for the entire clinic. For details, click here.
1. Click Clinic CC List. The LMR displays the list of current physicians associated with this patient. If the physician you want to add to the clinic CC list is not listed, you must first add the physician as previously described.
2. For each physician you want to assign to the note, select one or more of the following columns to designate how he or she will receive the note.
Note: The Dist. Prefs. column indicates the provider's distribution preferences if selected in Custom Preferences or in the Referring DB.
Print - When printed, notes include cover sheets with mailing addresses.
Fax - If no fax number is recorded for the physician, or if your practice does not have an authorization number for faxing, no check box appears in the fax column. All ten digits of the fax number must be entered.
Email - If no email is recorded for a physician, no check box appears in the email column for that physician. The full email address must be entered (example: woetest@partners.org).
Clinical Message - Indicates that a note for the specified patient is available. Clinical message can only be sent to providers within the Partner's network.
3. Click Ok to save your selections.
For LMR providers who have selected a preferred communications option (from Custom Preferences), a check box displays in the AN (auto notify) column. Click this check box to request that the associated provider receive a notification whenever a note is finalized for the associated patient.
A green check mark indicates a link to that physician's email.
Notes:
The AN (auto notify) option is only available for providers who have selected a selected a preferred communication option. This check box does not otherwise display.
Once a note has been finalized and distributed, it is no longer eligible for auto-notification.
If you delete a physician from this grid who is currently assigned to a CC list for an existing note, that physician's name is added to the "Additional CC List for this note" grid in the CC list window.
You are not allowed to delete a physician from this grid who is currently assigned to a clinic CC list. Clinic CC lists are used by practices using automatic note distribution.
List of past, current, and future appointments
The Visits section displays a list of appointments from practices using the LMR. Clinical laboratory values associated with past appointments can be displayed by clicking the code in the CDR (clinical data repository) column. For a paper copy, click Print.
The LMR displays retail and mail order pharmacies entered by you or other LMR users for this patient. The pharmacy indicated as the default is the one used when electronically sending prescriptions (although you can override this prior to signing). To add a new pharmacy, do the following:
1. Click New Pharmacy, and determine whether you want to look up information by name, name and city, city, phone number, fax number, zip code, or mail order. (Or click the Favorites button to select from your list of pharmacy favorites. See Working with Pharmacy Favorites for more information.)
2. Type information according to the lookup method selected and click Search (or press ENTER). Or for mail order pharmacies, just click Search.
3. Click the pharmacy name. (To add as the default pharmacy, click the associated check box before clicking the pharmacy name.)
Note: If the pharmacy you want is not found, click Add New to display fields for adding a pharmacy not in the database. Type information and click Ok (ALT+O) to update information. Be sure to click Feedback to send a message to the LMR support team to notify them of any missing pharmacy. Manually entered pharmacies are ineligible for ePrescribing, and are sent via fax only.
Updates can be made directly to the displayed information by typing over it and clicking Submit.
This section lists the patient's current consent status--that is, whether he or she has opted in or out of using the Massachusetts HIway to share health information with authorized health care organizations. Click Update Consent to update this status on behalf of the patient. Note that if you click Opt-in for the patient, you must also indicate that you have obtained written consent from the patient. This consent must be scanned into the LMR.
Note: Patients are also able to update their consent status using the Patient Gateway application.
