Editing Code Status Information (MGH Only)

Edit code status information, as follows. You can only edit information for your current location.

Note: To edit information for sites other than your current location, you must first log into a practice associated with that location (using the Location option from the Select menu).

1.   If you haven't already, select the patient and go to the ACP Summary module. Then, click the link for the Code Status document associated with your site.

2.   Click Edit.

3.   If applicable, update the option indicating Full Code (patient wants all life resuscitation treatments, with no limits), DNR/DNI, or DNR/DNI/Resuscitation Options (patient wants to limit treatments) by clicking the appropriate button. If switching from DNR/DNI/Resuscitation Options, any previous limits that were documented are automatically removed.

4.   If applicable, update previously documented limits by clicking check boxes.

5.   If the patient has completed the MA DPH Comfort Care/DRN State form, click the associated check box. Be sure also to update the date, as appropriate.

6.   To add additional information about this patient's End of Life discussion, click into the lower text box and type. To view sample text, click the link for examples on the right. To add sample text directly from any example, click the associated text.

7.   If the source document for DNR information exists as a note for this patient, click to identify the date of this note.

8.   Click Save.

9.   Click Sign (F10). The LMR indicates the changes that will be saved. Type your key and click Sign.

Note: For MGH users, a note is automatically created each time you enter or edit code status options for a patient
