1. If you haven’t already, go to the Care Plan module.
2. Click the link for the name of the goal to display all fields in edit mode.
3. Update fields for Target Date, Goal, and Instructions, as appropriate.
4. If this goal is associated to one or more problems on the patient's problem list, they are indicated following the "Problem" field. To update this association, click Add/Edit Problems. The LMR displays a pop-up screen with the patient’s active problems. Click to select or deselect the check box for one or more problems, and click Ok (ALT+O).
5. If desired, update fields for Barriers and Comment.
6. Click Ok (ALT+O) or Ok-Add New.
7. If you want to save your work now, click Sign (F10). The LMR indicates the changes that will be saved. Type your key and click Sign.
8. Depending on your set up, you may be prompted to select a cosigner, sign for a provider, or both. See Selecting a Cosigner and Signing and Saving for a Provider.
