Adding Multiple Doses of an Immunization

These steps are designed to allow you to add multiple historical doses of the same product or vaccine when detailed information about each dose is not available. Whatever action you select for the first dose on the form is applied to each associated dose -- you cannot select different actions for any of the multiple doses. Accordingly, if you need to document the current dose as "Done" and previous doses as "Done elsewhere," you should do this in two separate entries for the immunization. Otherwise, all multi entries are documented as "Done" rather than "Done elsewhere."

1.   If you haven't already, go to the Immunizations module.

Important: The LMR now checks to be sure you are logged into the correct location before allowing access to the New Entry or View/Edit screen for an immunization. You can no longer be logged in at the practice level if any subsets for that practice are live with MIIS (Mass Immunization Information System).

2.   Select the immunization you want to document. For details, click here.

Note: The LMR displays the full entry form only for one of the LMR doses that you are documenting. Only the product and the coded response will be propagated from that entry to the multi entries.

3.   In the Select Action section, select an option for all doses that you are documenting. For details, click here.

4.   Complete other fields for the first dose, as desired. For details, click here.

5.   Click Add Multiple (ALT+M). The LMR displays a list of up to four historical doses that you can add. (You can also click Add Multiple to add four more doses, if desired.)

6.   For each historical dose that you want to document, select the date. You can also type comments, if appropriate.

7.   To save this immunization record as a note, click Save as Note.

8.   Click Ok.

9.   If you want to save your work now, click Sign (F10). The LMR indicates the changes that will be saved. Type your key and click Sign.
