If you haven't already, select the patient and open the medications order form
If needed, for inpatients and ED boarders, initiate medication orders from the Pre-Admission Medication List (PAML).
Specify the medication.
Respond to interventions as prompted.
Set the dosing, frequency, infusion rate, and other parameters as required.
Fixed dose, adult (patient over 19 years old on admission)
Fixed dose, non-adult (patient less than 19 years old on admission)
Continuous medicated IVs, with or without bolus
Patient-Controlled Analgesia (PCA)—Adult
Patient-Controlled Analgesia (PCA)—Pediatric
Patient-Controlled Epidural Analgesia (PCEA)
Epidurals
Loading Dose
Taper dose (sequential days, applicable meds only)
Sliding scale (applicable meds only)
Heparin sliding scale (IV Cardiac or DVT/PE)
Repetitive Order Medications (predefined)
The Warfarin Dose Adjuster (WDA) is available if the selected medication is Coumadin (Warfarin Sodium), the administration route is PO, and the selected patient is an adult.
Click Warfarin Dose Adjuster (ALT+W).
Read the calculator message, then click OK.
Complete calculations as appropriate.
The calculator page and taskbar icon at the bottom are named "WARFARIN DOSE ADJUSTER"—if you click a link that opens a different page, click the taskbar icon to get back to the calculator.
Click in the upper right to close the calculator and any other unneeded pages.
Complete the dosing as appropriate (see options above) using the calculated dose.
Complete the order as applicable.
To order other meds, repeat the process of looking up the name and completing the order.
When all orders are written, click Cancel (ALT+C) to return to the Medication Orders scratch pad.
Depending on the order, you may be prompted to include consequent orders. See Responding to a Prompt for Consequent Orders.
Write additional orders as needed, then sign the session.
