Choose a Medication Reconciliation Tool—Application Portal

The Medication Reconciliation applications include the PAML Viewer, the Close Utility, and the PAML Builder. You can access the Viewer and the Close Utility from the PAML Portal. The Close Utility is restricted to authorized users.

Click a link to open an application. You must log in to use the application, then look up the patient record to continue.

The PAML Viewer displays the most recent pre-admission medications list by default, and allows you to display past lists. A list may be open (current), closed (at discharge, or manually if required), or errored (removed from the record as incorrect). Authorization is required to view errored lists. The PAML Builder is accessible from the following locations:

• MGH—Provider Order Entry application in the CAS (Clinical Applications Suite)

• BWH—BICS Order Entry

• Standalone—Partners Applications > Medication Reconciliation)
