If you haven't already:
Click Write (ALT+W).
Click VS/Monitoring/Restraints/Code Status (ALT+V).
Click Life Sustaining Treatment.
Complete the form
Limitation of Life-sustaining Treatment (B) is selected by default. Click or press the letter to select A - Full Code (discussed with patient/surrogate), or C - Full Code (discussion with patient/surrogate not appropriate or possible at this time).
All three options are also offered on signing the first set of inpatient orders, with full code "not discussed" as the default selection.
If full code "not discussed" is ordered, any MD, NP, PA, CNM, or medical student who writes orders for the patient will be prompted to update the order. The reminder is timed to appear one time only on the third day of admission for patients in intensive care, and the tenth day for all other patients.
For Limitation of Life-sustaining Treatment:
Read the note in the bottom frame. If needed, click Excerpts from Life-sustaining Treatment Policy (ALT+L) to view MGH Clinical Policy. Click Print (ALT+P) for a paper copy; click Close (ALT+C) to return to the order form.
Click (or press the corresponding letter) to select one or more limitations to methods for resuscitation.
For Other, type a description. Examples are listed beneath the option. If the text overflows the display area, click Zoom (ALT+Z) for a full, editable view. Click Close (ALT+C) to return to the order form.
Click OK (ALT+O).
The start time for this order is based on the time it is signed.
Write additional orders as needed, then sign the session.
