If you haven't already:
Click Write (ALT+W).
Click VS/Monitoring/Restraints/Code Status (ALT+V).
Open the restraints order form:
If the patient had a restraint order that expired the previous day, click Order (ALT+O) when prompted to write a new restraint order.
Click Restraints (ALT+T). If an active restraint order exists, click New Order (ALT+N) when prompted to discontinue the active order. Only one (1) active restraint order is allowed.
Indication: Click to select Non-Behavioral (Medical/Surgical) (ALT+N) or Behavioral Health Management (ALT+B). Complete the order for the selected indication as described below.
If you select Behavioral Health Management, read the warning, then click OK.. Select the Non-Behavioral option to change the indication.
Non-Behavioral (Medical/Surgical)—Complete the order
Non-behavioral restraint orders expire at midnight on the day after they are written.
If needed, click to uncheck the statement that less restrictive interventions were considered and/or attempted.
Click to select one or more behaviors indicating the need for restraint.
As appropriate, type to provide additional details (250 character limit).
Click to select one or more types of restraint: Bed, Torso, Chair, Mechanical Limb, Mitts, and/or Elbow Splints.
For Bed, Torso, Chair, and Mechanical Limb restraints, click to select one restraint from each category selected.
The available restraints options depend on the patient location and the reasons for ordering restraints.
PRN restraint orders are not allowed. The patient status must be evaluated by the ordering clinician and the restraint order written at the time that a restraint is needed.
Click OK (ALT+O).
By default, instructions are included that the RN can discontinue a restraint when the behavior requiring restraint is resolved. The RN should notify the responding clinician when restraint is removed, and request the order to discontinue. If this is not appropriate, click to uncheck the instruction.
Behavioral Health Management—Complete the order
If needed, click to uncheck the statement that less restrictive interventions were considered and/or attempted.
Click to select one or more behaviors indicating the need for restraint. Some options are not available for all locations.
Click to select a medical condition, behavioral condition, and response to the restraint.
Click the Yes option if the patient has sustained injuries, otherwise, click the No option.
As appropriate, type to provide additional details (250 character limit).
For all care units except Blake 11, type the number of hours the restraint may be used.
There is a maximum number of hours for any restraint, based on the reason and the patient age, as noted.
Click to select one type of restraint.
The available restraints options depend on the patient location and the reasons for ordering restraints.
Click OK (ALT+O).
Assessment—Complete the documentation
MDs, PAs, and NPs are reminded to complete the daily assessment when writing additional orders for a patient who has an active order for non-behavioral restraints. The reminder displays every day that the order is active, for every ordering provider, until the assessment form is completed.
The assessment is available online in Provider Order Entry.

Notes:
The start time for this order is calculated from the frequency based on the time it is signed.
The Restraint order status is displayed in the UCM for patients with active restraint orders.

Restraint orders cannot be added to transfer orders. The need for restraint must be assessed on the patient's arrival in the receiving care unit.
