March 12, 2013—DEx Discharge Summary Notes for Heart and Lung Transplant Patients
For patients with a discharge diagnosis of transplant of heart or transplant of lung, their discharge summaries in DEx (discharge express) is automatically set to the applicable type of note. (Providers can manually select a different note type if needed.) These notes, in addition to general information needed for all admissions, include sections for transplant-related concerns.
Key features:
Note type is set automatically based on patient's diagnosis for discharge (see Completing the Face Sheet, Primary Diagnosis)
The four (4) transplant diagnoses with unique notes are now heart, lung, liver, and renal transplants
Additional information includes general transplant data such as the type of graft, wound care, vascular concerns, episodes of rejection, immunosuppressive medications, infectious disease and prophylaxis, high risk donor, the mandatory multidisciplinary plan, clinical follow-up date, and care team contact information
For lung transplants, other information includes the type of transplant, ischemic time, acute graft dysfunction, ECMO, ventilator time, post-transplant length of stay, and studies, exams, and tests of lung function
For heart transplants, other information includes studies and tests of heart function

Discharge Order Printing
The function for generating paper copies of the face sheet, discharge orders, referrals, instruction, and patient discharge medication list (PDML)has been enhanced to make it easier to determine which forms go to the patient, and which go to post-hospital care facilities and providers.
Key features:
Discharge forms for printing are sorted into 2 groups: Printouts to be given to the patient, and Printouts to be sent to VNA/outside facility
The Post Hospital Patient Care Plan and the Patient Discharge Medication List (PDML) are listed in both sections
The PDML items include an information icon; hold the mouse pointer over the for details on saving, printing, and distributing the PDML
Automatic reminders on saving, printing, and distributing the PDML when printing other forms
