October 1, 2012—Enoxaparin
Effective today, Monday October 1st, the MGH is switching from dalteparin (Fragmin) to enoxaparin (Lovenox) as the sole low molecular weight heparin on the Formulary.
Order entry systems templates, order sets, and automated dispensing machine will be converted today. Existing orders for dalteparin will remain in place until the end of the week, at which point any remaining orders will be changed. Nursing in-services will be available on the Patient Care Units throughout today and tomorrow.
Questions should be directed to a pharmacist or unit nursing leadership.
