Writing Orders and Prescriptions for Services, Equipment, and Supplies for Discharge

Information from this section is carried over to the post hospital patient care plan in the nursing discharge instructions forms. See Discharge Orders and Instructions.

To help clinicians recall what will be seen by the patient, each field with information entered that prints on the Post-Hospital Patient Care Plan (PHPCP) is flagged with the

icon. Please keep in mind that information on the PHPCP should be patient-friendly, phrased to be easily understood by people who don't have medical training.

Face Sheet Discharge Orders and Instructions are not available in the newborn nurseries. You cannot access the forms, even if you select a patient from the associated maternity unit lists or from patient lookup.

Select the patient and click Face Sheet Discharge (ALT+F) or Discharge (ALT+G).
If needed, click the Services folder to access the form.

Click the corresponding checkbox in the Sel column for each service needed.
Type the name of the service (if not listed), the frequency, and any comments in the appropriate columns. Press TAB to move forward through the fields quickly.

Click the corresponding checkbox in the Sel column for each equipment or supply needed.
Type the name of the equipment or supply (if not listed), the frequency, and any comments in the appropriate columns. Press TAB to move forward through the fields quickly. To work in a larger text area, click the field, then click Zoom. To return to the order, click OK.

Click the corresponding checkbox in the Rx column for the order needing a prescription.
Complete the prescription:

For services other than nutrition:

Type to provide a diagnosis or reason for the order.
If needed, type to include comments or additional information or instructions.

For nutrition, the MGH Nutrition & Food Services prescription form opens in a new window. To generate a paper copy:

Click the heading in red at the top of the page.
When prompted with printer options, set as needed, then click OK.
Click in the upper right of the browser window to close it.

For equipment and supplies:

Click to check one or more pieces for the prescription.
If the item needed is not listed, click to check an Other (non-meds) option. When prompted, type the name of the piece, then click OK (ALT+O).
Type the type, product, size, quantity, and comments, as applicable, in the appropriate columns. Press TAB to move forward through the fields quickly.

Click OK (ALT+O).

Click the line for the service, equipment, or supply with the prescription you want.
Click View/Edit Rx.
The prescription form opens. Edit as needed, then click OK.

Click Print Rx.
If you added, edited, or deleted prescriptions for services, equipment, and supplies, type your signing key when prompted, then click OK (ALT+O).
Click to uncheck or check items on the list until the prescriptions you want to print are checked.
Click Print (ALT+P).
Click Close (ALT+C).

If you would like to check your spelling, click Spell Check. When you have completed the check, click OK to close the Spell Check complete message.

Complete the Face Sheet, Medications,and Order Info forms if applicable. See also: Instructions for Discharge Orders and Instructions.

Save the changes, and finalize the orders if the patient is ready for discharge:
