Saving to the shared file area (SFA) 

  • If you haven't already:
  1. Click Referrals Received on the main menu.
  1. Click the patient name to select a case.
  1. C lick details on the Referral Management view.

Saving referral information to the shared file area (SFA)

Saving All on SFA is restricted to  Mass General Brigham Home Care. Clinical documents, attachments (from case), and the pre-admission analysis (PAA) form are saved.

  • Click save all on sfa.
  • All clinical documents for the selected patient are saved to the HH_SWEEP shared file area.
  • The file name is LastName,FirstName_visit (SOC, ROC, Revisit)_visitExpectedDate. 
  • Once the save is successful, the button is disabled.
  • If the save is not successful, you'll receive an email. You must manually save the file to the SFA.
  • For a patient at a Mass General Brigham site, the file includes:
  • Summary of the referral, including the face sheet referral detail
  • When shared, one or more of the following clinical documents from Epic - the Facesheet, Referral Summary, Medications, Operative notes, PT/OT/ST Notes, Case Mgmt Notes, Discharge Info, and/or Early Maternal Discharge Form
  • Case attachments
  • Pre-Admission Analysis form
  • For a patient NOT at a Mass General Brigham site (OUT), the file includes:
  • Summary of the referral, including the face sheet referral detail
  • Case attachments
  • Pre-Admission Analysis form

Returning to the patient's referral management view

Click manage in the upper right.
