Any text entered into the Comment field on the Problem entry form displays here in black. Click the link for this problem in the Problem Description column to view this text, plus the structured notes, in its entirety.
Any
text displayed in green represents text pulled in from the associated
structured note for this problem as entered by any provider. Click
the 
icon to view this problem text in its entirety.
Note: If there is more than one structured note for a problem, the problem text entries will display in reverse chronological date order, with the most recent shown first.
Entries preceded by "FH" indicate entries from the family history list.
