The Diabetes Report identifies your panel of patients who are diagnosed with diabetes. This report also provides clinical information relevant to patient care for diabetes based on HEDIS criteria. This information includes lab values (HbA1C, LDL, and microalbumin:creatinine), routine care exams (foot exams, eye exams), vital signs, whether or not aspirin is on the patient's medication list, and indication of smoking status.
Note: The medical director view of the Diabetes Report displays the practice panel of patients with diabetes and the same relevant clinical information as the provider view. In addition, the medical director view shows percentage of diabetes patients with HbA1C>7 and HbA1C<7 for the practice.
To access the Diabetes Report, go to Report Central from the Reports menu, and click the associated link.
