When you order a medication in LMR, the system checks to see if the patient has renal impairment by calculating the creatinine clearance value.
If there is a medication in the Nephros Knowledge base with a drug
substitution warning and/or dose and frequency adjustments, a special
dosing flag displays
in the medication lookup next to the medication. When you place your
cursor over the flag, it also identifies the medication as Nephros.
When LMR displays the drug substitution warning, your options are:
Select the suggested substitution.
Continue the order for the medication that prompted the Nephros alert and provide an override reason.
Cancel the order.
If the medication being ordered is in the Nephros Knowledge base, and the patient's height and weight has been recorded within the last 12 months, you will receive a non-interruptive warning on the Rx prescription pad that lets you know the dose and frequency has been adjusted for the patient's renal function based on the calculated creatinine clearance value.
Click the see details link next to the Renal Dosing alert on the Rx Prescription pad. A popup will display the patient's creatinine clearance and calculation based on serum creatinine, height, weight, age and sex.
You can make changes or complete the prescription as is with the adjusted dose.
