The Discharge Reconciliation process can help ensure that the LMR medication list is kept up-to-date for patients who have had recent hospital admissions.
If the patient has been discharged from the MGH or BWH within the last 6 months, a Discharge Reconciliation button displays at the top of the Medications screen. (There are two exceptions: BWH discharges since 5/30/2015 that have been completed in Epic and if the list has already been reconciled.)
Upon seeing the patient for the first time following a hospital discharge, the provider clicks this button to display a table comparing the current LMR medication list (on the left) to the medications included on the patient's discharge list (on the right). Medications display as follows:
Medications are sorted by therapeutic class.
Medications that match between discharge list on the right and the LMR list on the left display in the same row. Rows are highlighted in pink if medications match with differences in dosing. Rows are not highlighted if there are no differences in dosing.
Medications that cannot be matched display in a row by themselves (such as new or discontinued medications).
The provider can either replace the entire list, or update individual medications. The provider can also indicate that the button should continue to display for future sessions (Reconcile in Part) or indicate that he or she has completed reconciling medications and the button should no longer display (Reconcile in Full). Clicking any of these buttons displays all associated medication alerts before returning the user to the main Medications screen.
Important: This process is for reconciliation,
not prescribing. If you need to generate a prescription for any reconciled
medication, you must do so by editing the medication using standard
LMR processing.
