Note: The following applies only if both of the following are true:
Your practice is participating in the PACCT study.
The patient has active journal entries (submitted using the Patient Gateway application).
1. If you haven't already, go to the Problems module.
2. Click Diabetes Journal.
3. Click Start Diabetes Journal Note to open the free-form notes entry page. The LMR automatically pre-populates that note with the patient's diabetes journal data. The subject of the note is automatically set to "Diabetes Journal Note," but you can change this or any of the other settings.
4. To bring in other patient journal topics into this note. Click the -Pt Journal- drop-down and select the journal data you want to include in the note. Click All to include every journal topic submitted by the patient.
5. To close the diabetes journal topic, click the associated button. The LMR displays a verification message. Click OK.
6. If you want to save your work now, click Sign (F10). The LMR indicates the changes that will be saved. Type your key and click Sign.
