Processing Journal Entries for Allergies

Note: The following applies only if both of the following are true:

• Your practice is participating in the PACCT study.

• The patient has active journal entries (submitted using the Patient Gateway application).

1.  If you haven't already, go to the Allergies module. Patients with active journal entries include the following:

• a link to the journal is indicated by the letter J.

• the date of the journal.

• a column for Patient Comment, added to the Allergy list.

• a secondary list of Other Journal Allergies, including items to be added or activated from the current list (if applicable).

2.   Do one or more of the following (based on journal entries):

• add allergies

  a.   Click Add New in the far right column of the Other Journal Allergies list. The LMR displays the Add New Allergy screen, pre-populating the allergen name based on the patient's journal entry. All additional information regarding this entry, including type of allergen (drug, food, or environment) and reaction are displayed in the Pt.Journal field.

  b.   Click to select the type of allergen.

  c.   For drug allergens, click Drug Lookup to find matching entries in the drug dictionary based on the patient's journal.

  Note: If no entries in the dictionary match what the patient has indicated, you can either click Take as Typed to accept exactly what was entered, or click Cancel to return to the search field to modify the entry.

  d.   Click to select the Reaction, as indicated by the patient.

  e.   Complete the Comment field, as desired. If you want to retain any other patient-supplied information, you must copy it from the Pt.Journal field before completing your entry. Once signed, the Pt.Journal field is no longer displayed.

  f.    Click Ok (ALT+O).

• edit allergies

  a.   Click the allergy name to display the entry in Edit mode. All journal information for this entry is displayed in the Pt.Journal field.

  b.   Make updates, as desired. If you want to retain any patient-supplied information, you must copy it from the Pt.Journal field before completing your entry. Once signed, the Pt.Journal field is no longer displayed.

  c.   Click Ok (ALT+O).

• activate any previously removed allergies

  Click Activate in the far right column of the Other Journal Allergies list.

  Note: This can only occur if the associated allergy was discontinued after the journal entry was submitted by the patient.

• save the journal as a note

  a.   Click the J to display the journal in a new window.

  b.   Click Save as note.

3.  To close the allergies journal topic, click the associated button. The LMR displays a verification message. Click OK.

4.  If you want to save your work now, click Sign (F10). The LMR indicates the changes that will be saved. Type your key and click Sign.
