This module allows an LMR user to display, accept, and reject patient-entered data. This data can come from a variety of sources, including from interactive forms (submitted through Patient Gateway), from a kiosk, or over the phone.
The Custom Visit Forms module provides all LMR practices with the ability to create forms that can be used to collect data from patients before a visit. When sent to patients with Patient Gateway accounts, they can send completed forms back to you through the Patient Entered Data module in the LMR. This can make it easier for you to review and update patient records based on this information. For details about creating Custom Visit Forms, click here.
Note: The LMR moves data older than 6 months to the Archive tab. To act upon data from the Archive tab, you must first unarchive it.

Use of standardized pediatric screening forms to improve patient visits. These forms include the Asthma Control Test, Pediatric Symptom Checklist (PSC), and the NICHQ Vanderbilt Assessment. If accepted, information is available in the Notes and Flowsheets modules.
Use of the LMR Family Social History module once it contains family history data entered by patients. Patients involved in this study enter family history data using the Surgeon General's Tool (SGT), which can be accessed via Patient Gateway or kiosk, or they can answer SGT questions over the phone.
Use of an automated telephony application for collection of patient information related to smoking status. A tobacco coordinator associated with the practice is responsible for determining if the information is to be accepted, rejected, or edited. If accepted, information is filed to the Health Monitoring module.
