Select the date associated with the onset of this problem. Click the calendar symbol to display a calendar for selecting a date. If not sure of the exact date, click the Approx. check box, and type an approximate date (you can use YY, YYYY, MM/YY, MM/DD/YY, or MM/DD/YYYY format). Otherwise, type an exact date in MM/DD/YYYY format. Click T to select the current date.
Note: You can also specify a date based on its proximity to the current date (T+ or T- a number of days). For example, to indicate tomorrow, you can type over the current date with "T+1." To indicate the day before yesterday, you can type "T-2."
