Adding OB Forms

• Select the appropriate provider from the OB/Midwife drop-down list. The list displays all providers in your practice. Note that this is a required field to utilize OB Reports.

• Enter any other applicable information. For fields requiring a search, type at least three letters of the word in the associated text box, and click Search.

• When you are finished, click Ok (ALT+O) or Ok Next-Pregnancy Dating (ALT+N), as desired.

Note: If the patient had a previous pregnancy entered in the OB Record, demographics and mother's details are copied from the previous pregnancy and can be edited as necessary.

• Enter applicable information.

• Choose at least one method (LMP, initial exam, ultrasound, perceived conception, or IVF) for determining the initial EDD. Select the associated button on the right to signify it is the initial EDD.

• Once a final EDD is entered (recommended), any edits to that date require a reason.

• When you are finished, click Ok (ALT+O) or Ok Next-Menstrual History (ALT+N), as desired.

Enter applicable information and click Ok (ALT+O) or Ok Next-Medical History (ALT+N), as desired. Note that the pre-pregnancy height and weight are required to calculate the weight change on the OB flowsheet.

Note: If the patient had a previous pregnancy entered in the OB Record, her menstrual history data is copied from the previous pregnancy and can be edited as necessary.

Enter applicable information and click Ok (ALT+O) or Ok Next-Genetic History (ALT+N), as desired.

• For items on the Medical or Surgical History list

  1.   Select the associated check box to designate an item as Current (Active), Past (Inactive), or Negative (Inactive).

  2.   Designate an item as OB-related by clicking the associated OB check box. If the item is also designated as Current, it appears on the OB-related Problems list.

  3.   Enter comments for an item as applicable.

• For problems that are not on the list

  1.   Click Add Problem. The problems search screen opens.

  2.   Type at least three letters of the problem name and click Search.

  3.   Add the problem. For more information, see Adding Problems.

  4.   Click Ok when you are finished to return to the Medical History form.

  5.   Designate the item as Current, Past, or Negative; flag the item as OB-related; and enter comments, as applicable.

• For procedures that are not on the list

  1.   Click Add Procedure. The procedure search screen opens.

  2.   Type at least three letters of the procedure name and click Search.

  3.   Add the procedure. For more information, see Adding Procedures.

  4.   Click Ok when you are finished to return to the Medical History form.

  5.   Designate the item as Current, Past, or Negative; flag the item as OB-related; and enter comments, as applicable.      

Note:  An item will default to a negative status if you do not designate an item as current, past, or negative, but you add comments or flag the item as OB; or if you designate an item as current, past, or negative, and then deselect the status of the item.

Enter applicable information. Click to select the Select all No check box and make any changes, if desired. Selecting a Yes response deslects the Select all No check box. When you are finished, click Ok (ALT+O).

Notes:

• If the patient had a previous pregnancy entered in the OB Record, her genetic history and ethnicity data is copied from the previous pregnancy and can be edited as necessary.

• If Select all No is selected, it appears on the Individual Patient Report.

Enter applicable information and click Ok (ALT+O).
