Generating Form Reports

To generate a report, do the following:

1.    If you haven’t already, select Form Reports from the Admin menu. The LMR displays the list of Outstanding Forms.

2.   If you wish to work with the Summary report, click the associated link.

3.   Click Create New. The LMR displays a Select Parameters pop-up box, prompting you to specify the date range for the report.

4.   Type or select the From and To dates, and click Generate. The LMR adds an entry in the Form Reports table for the report, with a status of “Pending.”

Note: Depending on the date range you selected, it may take some time to generate the report.

5.   Click to update the status to “Available,” when applicable.

6.   To view details, click link for the date range.

 7.  For a paper copy of the displayed report, click Print.

8.   To delete any report, click Cancel to return to the main screen and click .
