Problem-based plans guide you through the process of documenting the assessment and plan portions of a traditional SOAP note when adding a free-form or structured note.
1. Clicking Problem Based Plan from the Pt. Data tab displays a pop-up window listing existing patient problems and common problems. Click to select the problem you wish to create a plan for, and click Select. Or, to add a plan for a new problem, use the Problem Lookup button. (If you are doing both, click Select to add "checked" problems before clicking Problem Lookup. Otherwise, selected problems are not included.)
2. Click the name of that problem to select it. The LMR displays the Problem Based Plan form.
3. Complete fields to document the plan. If you are adding a plan for a new problem, you can also add that problem to the patient's problem list.
4. To include medication changes as part of the plan, click the associated link.
If the plan involves adding a new medication, click Add New, and add the medication using standard LMR processing.
If the plan involves editing an existing medication, click the medication name to display the entry form. Make changes using standard LMR processing.
If the plan involves discontinuing an existing medication, click the check box next to the medication name and click Discontinue, select the reason, and click Ok.
Click Ok after making medication changes to return to the initial form. The problem-based plan includes a detailed description of all medication changes.

plans include the icon (from
the Summary and Problems screens). Clicking this icon provides a direct
link to the associated plan.
