The Health Form report allows practices to provide patients with reports on demand, based on information in the LMR for camps, schools, and day care centers. For patients with Patient Gateway accounts, the most recent saved report is available under the Health Record menu.
Note: You can only access reports saved since November 4, 2011.
When initially displayed, the module displays a listing of previously saved reports, sorted by the date saved. If no Health Form reports have been saved, the list is blank.
Once generated, the Health Form report includes a summary of the patient's allergies, family history, conditions (problems), procedures, active medications, and immunizations, and details from the most recent physical exam. The report also includes the most recent TB risk assessment entered into a structured note and postural screening assessment. If desired, you can suppress specific information from report printouts by deselecting the check box to the left of the item.
The lower portion of the form provides check boxes for the provider to assess the impact of how this patient's conditions might affect his or her educational experience. You can also either attest to the patient's good health or provide details for how activity should be restricted.
