Family and Social History Overview

This module allows you to display and update the selected patient’s family and social history.

Family History information is useful to help identify patients at increased risk for diseases with a genetic or familial basis. The Fam/Soc History module captures family history information from clinician input in LMR needed for genetics-related decision support. In addition, family history data entered by a patient via Patient Gateway and approved or edited by the patient’s provider is also included. When applicable, a risk assessment may also be displayed based on the data entered.

There are four filter buttons available on the Fam/Soc History module:

• All

  Allows users to enter coded information on which computerized decision support can be based.

  Note: The LMR displays this view by default when you go to the Fam/Soc History module, unless you change it using Custom Preferences.

• Positive

  Allows you to view a filtered view of positive family history items for this patient, as well as the associated relative or relatives.

• Quick Entry

  Allows you to enter family history information with a single click.

• Uncoded Family/Social History

  Click the Uncoded Family/Social History link to display Social History information as well as Family History text entered via Notes. You can also click the Social History label on the Summary module to navigate directly to the Uncoded Family/Social History screen.

To print a copy of the patient's family history, click Print.
