Editing Visit Details

• To edit previously entered vital signs, go to the Flowsheets module. See Editing Flowsheet Values for details.

• To edit a previously documented administration (for a route, pre-medication, chemotherapy, other medication, blood product, or hydration), click here.

  Note:

  • You cannot edit previous administration values for Take Home Oral Chemotherapy medications. The LMR assigns values based on the associated dose for each visit.

  • You are not allowed to edit administrations for removed/discontinued routes, medications, blood products, or hydration.

  1.   If you haven't already, click the section on the Infusion Flowsheet for the value you want to edit.

  2.   Find the column for the correct visit, and click the value. If it has been more than 24 hours since the visit was created, the LMR displays, "You are charting on a past visit. Do you want to continue?"

  3.   Click OK. The LMR displays the entry form for that section.

  • To add a new administration for that section, complete the form and click Add (or Add/Update for Routes).

  • To edit a previously entered administration, select the row for that administration at the top of entry form. Update fields, as necessary, and click Update (or Add/Update for Routes).

  4.   Click Ok (ALT+O).

Note: You cannot edit the name of a route, pre-medication, chemotherapy, blood product, or hydration. Fix errors by discontinuing and adding correct entries.

• To discontinue a route, medication, blood product, or hydration solution, click here.

  1.   If you haven't already, click the section on the Infusion Flowsheet for the value you want to edit.

  2.   Click the name of the route, medication, blood product, or hydration solution in the left column. The LMR displays current information.

  3.   Click the associated button. The LMR displays a verification message. Click OK.

  4.   Click Ok (ALT+O).

• To edit intake/output, administration complications, extravasation follow-up, teaching, discharge disposition, or nursing notes, click here.

  1.   If you haven't already, click the section on the Infusion Flowsheet for the value you want to edit.

  2.   Find the column for the correct visit, and click the value. If it has been more than 24 hours since the visit was created, the LMR displays, "You are charting on a past visit. Do you want to continue?"

  3.   Click OK. The LMR displays the associated entry form for that section, with previously entered values (if applicable).

  4.   Update information, as needed.

  5.   Click Ok (ALT+O).

Note: The Infusion Flowsheet flags sections that have been edited after midnight of the day following the visit date as follows:

E - Indicates that information for the associated section was edited.

A - Indicates that information was added to the associated section (such as a new administration).

R - Indicates that information was changed more than 6 months after the visit was created.
