Custom EOV Forms Builder Overview

The Custom EOV Forms Builder allows you to create any of four different types of forms used with the EOV (end of visit) processing:

• Summary

• Pt Summary

• Billing

• Order Requisition

These forms help facilitate complete, consistent, and correct billing for services and care rendered.

Forms can be linked to a specific practice or saved for general use by all LMR users. Order Requisition forms are linked to specific resulting labs.
