1. If you haven't already, click the EOV Header button on the EOV Form Builder module.
2. If necessary, use the filter buttons to locate the header that you want to use for your new EOV form header. See Using Custom Filters for details.
3. Click the header name to display it in Edit mode.
4. To update any text centered across the top of the header, click into the Top text field and type. For example, you might want to use this field to indicate the type of form.
5. To include any additional identifying information (for the patient, encounter, or provider) in either the left or right column of the header, click one or more items from the Available items list, and then click the on the left or right side of the screen.
6. To remove any identifying information (for the patient, encounter, or provider) from the left or right column of the header, click item or items and then click the associated button.
7. To move identifying information in either the left or right column of the header, use the buttons in the center of the module. Use the or
button to move items between columns. Use the
or
button to move items either up or down in the list.
8. To update any text centered across the bottom of the header, click into the Bottom text field and type.
9. Click Ok (ALT+O).
10. Type a name for your new header. Then, select whether you want to save it to your personal list ( My List) or make it available to others within your practice ( Practice). If desired, select Save to Modules.
11. Click Save As New.
