Creating an EOV Form

Note: If the EOV form you want to create is similar to an existing one, you can save time by editing an renaming that form.

1.   If you haven't already, go to the Custom EOV Form Builder module.

2.  Click the  at the Add New... field and select the type of form you want to create. Make additional selections according to the type of form you choose.

• Summary

  • Click the at the Section field to select EOV data that you want to include. For each section you selected, add fields that you want reported. Do so by clicking the section name to display a picklist of associated fields. Then, click checkboxes, as desired. You can use the Labels and Text options to add formatting (such as bold, italics, and underlining) between sections on your form.

  • To remove previously selected items, click . To reorder items, click the up or down arrows in the Move column.

  • If you have previously created a header that you want to use for this form, click the  at the Header field and select that header. Otherwise, no heading is used. See Working with Headers for EOV Forms for details.

• Patient Summary

  -   Click the at the Section field to select EOV data that you want to include. For each section you selected, add fields that you want reported. Do so by clicking the section name to display a picklist of associated fields. Then, click checkboxes, as desired.   -    To remove previously selected items, click . To reorder items, click the up or down arrows in the Move column.

  -   If you have previously created a heading that you want to use for this form, click the  at the Heading field and select that heading. Otherwise, no heading is used. See Working with Headers for EOV Forms for details.

• Billing

  -   Click the at the Section field to select EOV data that you want to include. For each section you selected, add fields that you want reported. Do so by clicking the section name to display a picklist of associated fields. Then, click checkboxes, as desired. You can use the Labels and Text options to add formatting (such as bold, italics, and underlining) between sections on your form.

  -    To remove previously selected items, click . To reorder items, click the up or down arrows in the Move column.

  -   If you have previously created a heading that you want to use for this form, click the  at the Heading field and select that heading. Otherwise, the no heading is used. See Working with Headers for EOV Forms for details.

  -   Click the at the Billing field to select the institution that this billing form is to be used for.

• Order Requisition

  -   Click the at the Section field to select EOV data that you want to include. For each section you selected, add fields that you want reported. Do so by clicking the section name to display a picklist of associated fields. Then, click check boxes, as desired. You can use the Labels and Text options to add formatting (such as bold, italics, and underlining) between sections on your form.

  -    To remove previously selected items, click . To reorder items, click the up or down arrows in the Move column.

  -   If you have previously created a heading that you want to use for this form, click the  at the Heading field and select that heading. Otherwise, the no heading is used. See Working with Headers for EOV Forms for details.

  -   Click the at the Lab field and select whether this will be an internal lab for MGH or an external lab.

3.   Click Save.

4.   Type an EOV Form Name.

5.   Click to select whether you want to save your EOV Form to your personal list (My list) or make it available to others within your practice ( Practice or Practice General). Regardless of which option you choose, other LMR users cannot edit or delete your EOV form.

6.   If desired, click Save to Modules.

7.   Click Save.
