1. If you haven't already, select the patient and go to the Infusion Flowsheet module.
2. If the patient already has a flowsheet and you wish to begin a new one, click Create a New Flow Sheet (ALT+N).
Note:
The LMR prevents you from creating a new flowsheet if one already exists for your institution.
Required fields are indicated by red asterisks.
3. The LMR assigns a name to the flowsheet based on your location. You can edit this name by typing over the Flowsheet Name field.
4. Click to select the primary diagnosis from the drop-down list. (You can add additional diagnoses, if appropriate, during step 6.)
5. Click to select an MD and Nurse from drop-down lists. If appropriate, you can also select a Social Worker and Fellow/NP.
6. If the patient has more than one diagnosis, click Add DX. Select a Diagnosis and MD for each diagnosis. Be sure that the Primary option is selected for the primary diagnosis.
7. You can also enter general comments (such as to identify if the patient is a diabetic) by typing into the text area. Do not enter information here that belongs in other sections of the flowsheet, such as administration complications and extravasation notes.
8. Click Ok.
If the patient did not previously have a flowsheet, the LMR displays the new visit screen. For details on completing this screen, click here.
Otherwise, the LMR displays the initial Nursing Oncology Flowsheet selection screen.
