1. If you haven't already, go to the Oncology Form Builder module under the Custom menu.
2. Click Add New.
3. Type the text that you want to include in consent form. If desired, use custom paragraphs and system macros.
4. Use the Select Field drop-down list to include treatment plan fields.
5. To format text, use the special formatting buttons.
6. When you are finished, click Ok (ALT+O).
7. Type a template name.
8. Click to select Practice.
9. Click Save.
10. On the list screen, click to select the Default to Print option to set the form as the default. There must be a default form set in order for the form to print.
